How to sign up for IM590
**The Office of Student Services issued a memo on January 14, 2005, changing the way we sign up for electives. The appropriate changes have been made below in order to comply with the changes. Should you have any questions, contact Dr. Falls**
The ER shadowing opportunity is a college approved elective with the Department of Internal Medicine. The course is designated IM590, section 001. Normally it is a one-credit elective, which requires a minimum of four hours of shadowing in the ER.
The first thing you need to do is to make sure all your immunizations are up to date. This is very important, as Dr. Falls has stressed this at several of our meetings! Visit https://ntweb11.ais.msu.edu/HCPSTUImm/ to access the Healthcare Professional Student Immunization Database. Once there, click on "Meets CDC Requirements" to print out proof of immunizations. It is necessary to bring this print out with you in order to get the forms needed for the elective!
Next, visit Cathey Pepper in C-103 Fee Hall. Give her your proof of immunizations print out and tell her you would like to sign up for the ER Elective. She will give you an "Application for Directed Study" form, a "Permission to Shadow a Physician in the Hospital" form, as well as a purple "Late Add Form" if it's now past the deadline for students to add classes to their own schedule themselves. Fill out all the forms, making sure to indicate the dates on which you intend to shadow on the Directed Study form.
The purple "Late Add" form goes back to Cathey (if you were given one), and you must now visit the Department of Internal Medicine (B-305 of West Fee Hall) with your completed "Directed Study" form.
At the Department of Internal Medicine, obtain signatures from the department chairperson (Dr. Hayes) and the instructor of record (Dr. Hughes). Next, take your completed, signed Directed Studies form, as well as the "Permission to Shadow a Physician in the Hospital" form to Dr. Falls' office. There, Dr. Falls will fill out the top half of the "Permission" sheet.
The next step is to take the "Permission to Shadow a Physician in the Hospital" form and the "Directed Studies" form to the Medical Education Office at the hospital in which you wish to shadow a physician. There, the appropriate representative will look over your "Directed Studies" form and then sign the "Permission" form, giving you permission to shadow in the hospital on the date(s) indicated. The hospital should keep the original of that form and copies should be returned to the Office of Student Services.
After all the paperwork is completed and approved, return the signed "Directed Studies" form and signed copies of the "Permission to Shadow a Physician in the Hospital" form to the Office of Student Services. If all forms are signed and approved, Dr. Falls will then sign the "Directed Studies" form, allowing you to register for the shadowing experience.
Don't forget that you must register for any elective course no later than the middle of the semester.
